The philosophy of surgical management of hypertonia in cerebral palsy (CP) entails weighing the pros and disadvantages of various treatment modalities and selecting the best approach based on unique requirements and patient goals. Physical therapy, drugs, injections of botulinum toxin, and in extreme situations, surgery, may all be used in this situation. In these situations, surgery is performed to improve function, lessen discomfort and spasticity, and improve the patient’s quality of life. The type of surgery chosen depends on the patient’s age, concomitant conditions, and personal goals in addition to the location and severity of the hypertonia. The surgical strategy should always involve feedback from multiple disciplines.
